If you are new to Island Kidney Care, aloha and welcome. We want to make your first visit as smooth and comfortable as possible. This page provides important information and forms to help you prepare for your appointment.

Our team works together across the Hawaiian Islands to provide coordinated, personalized kidney care, so you can feel confident knowing your care is supported from your first visit forward.

Before Your Appointment

Please arrive 15 minutes before your scheduled appointment to allow time for check-in and any additional paperwork.

At your first visit, you will be asked to complete and review:

• Consent for services

• Authorization to release health information

• Acknowledgment forms

• Notice of Privacy Practices

You will be provided a copy of these documents upon request.
